Tammy Miller-Niles – Executive Director
Tammy has been with Live Free Home Health Care since November of 2010, but began caregiving while a student in high school, spending overnights with an elderly dementia patient and eventually becoming the primary caregiver for her grandmother in her own home. She spent six years with a home care agency as Client Care Coordinator before joining Live Free Home Health Care.
Tammy is a Certified Dementia Practitioner, and in her spare time, she enjoys hiking, kayaking, baking, and being with family.

Christine Connolly − Care Coordinator
Christine joined the Live Free Home Health Care team in 2016. She began as a caregiver, but quickly transitioned into the office to provide administrative support. Christine is responsible for hiring, training and orientation of new staff. She also strives to find suitably matched, quality caregivers for our clients and understands the importance of building a successful team.
Christine has a background in business management and is a mother, or mother figure, to nine wonderful children. They all enjoy a variety of activities ranging from arts and music to sports. In Christine’s spare time – oh wait, she doesn’t have any.

Patti Noyes-Oakley, LPN, DCP – Nurse Care Manager
Patti joined Live Free Home Health Care as an LPN in 2009, bringing with her more than 25 years of medical experience. In 2021, Patti earned her certification as a dementia practitioner. Patti says, “Being able to help a client in the home setting is very rewarding. It offers me the chance to interact with our clients in ways that you can’t in a doctor’s office or a nursing home. It’s the family atmosphere that keeps me here. I was born and raised in the Plymouth area, and this makes it easy for me to connect to our clients. Having something in common with each of my clients is something that, for me, makes the difference in my success as a nurse.”
In her spare time, Patti enjoys gardening, kayaking, and long-distance walking. She is a 20-year member of her local fire department in Hebron, and she is a Nationally Registered Advanced Emergency Medical Technician. She is also a proud U.S. Army Veteran, having been stationed throughout the US, Germany, and Korea.

Nicholas “Nick” Miller, Care Coordinator
Nick is the friendly face you see when you walk into the door at Live Free Home Health Care. He has years of experience in both customer service and administrative work. As a Care Coordinator, Nick takes the time to get to know each individual client and to become familiar with their needs. He also works closely with our caregivers, getting to know them on a personal level in order to make the best possible client/ caregiver match. Additionally, Nick manages the inventory of supplies for the office and assists with payroll.
Outside of work, Nick enjoys time with his family and his dog. Nick almost always has a smile on his face and he is eager to help in any way possible. He also loves snacks, so feel free to drop some off anytime.
